Continuity of care is effective – for caregivers and adults in palliative care
Being able to work with and support families prior to the loss and then continue working with them with check-ins throughout the first year after the loss, and offering one-on-one support as well as group support has been met with positive feedback. It has also been helpful to speak with parents right after the loss about the types of supports we offer after the support from their direct communities starts to dwindle. Having groups, one-on-one support, and events throughout the year is comforting for parents who are just starting to look down the long road of grief. Being able to offer the knowledge that they are not alone is a powerful element to this kind of support.
