Extraordinary Client Care from the Philip Aziz Centre for Hospice Care
“I would like to send my compliments for a PAC Community Program Client Care Coordinator. Sarah has been an integral part of our palliative team, always prompt to respond to our patients’ needs, and works so collaboratively with our team. I am writing to highlight a recent event where she supported our patient with extraordinary compassion, thoughtfulness, patience, and persistence.
Mr. X was a patient our ours, who, unfortunately, had [complex advanced medical and palliative care needs]. He was very isolated, all alone, in Toronto. His sister had arranged for him to go to B.C. so she could support him there, but he had to travel alone. With his [medical] history and challenges, he had trouble following any two-step tasks, and he had missed a flight before. I originally asked Sarah to assign a volunteer to bring him to the airport; but (after knowing about Mr. X’s complex needs) she offered to accompany him to the airport, personally. With a letter provided by his MD, and with her persistent advocacy for him at the airport’s security, she accompanied him all the way to the gate and waited for him until the plane departed. There were many hiccups along the way, … which she helped support him through. Despite the patient’s memory and executive functioning challenges, he managed to call my office to express his heartfelt appreciation for all she had done for him.”
