Who can make a referral?: We accept referrals from Home and Community Care Support Services (formerly Community Care Access Centers /CCAC, or LHIN), health care professionals and hospitals, community support agencies, family members, friends or anyone living with a life-limiting illness within our boundaries.
Where we offer supports: Philip Aziz Centre serves the Toronto community bounded roughly by Eglinton Ave to the north, Warden Ave to the east and Jameson to the west, south to the Toronto Islands. Our Emily’s House @Home program serves children living within the “M” area code.
What happens after a referral: In response to a request for hospice care, our client care coordinator will connect with you to arrange a visit to conduct an admissions assessment. Together, we’ll discuss what is important to you and what would help you most. We’ll review our hospice supports and develop a personalized care plan.
Some terms of service: Hospice services are provided free of charge and are contingent upon meeting our admissions criteria. Our services are based on resource and service provider availability.
